Patient’s Registration Card

Supplied by Silimed, the patient’s registration card is to be filled in with data such as the date of the operation, the complete name and signature of the surgeon, and the serial, reference and volume numbers used on each implant.

When duly filled in, the card facilitates updating the SILIMED registers and tracing the product after implantation. Yet another guarantee of safety offered by the company.

Patient’s Card

For the purpose of offering further safety, each box of our implants comes with a patient’s card to which the surgeon or his team attaches labels with the serial, reference and volume numbers used on each implant.

This card should be carried in the patient’s wallet in case it is needed for examinations such as mammography and bone mineral densitometry.
